Fortune telling room is the only place I've found it, but it has a very low drop rate...

It never dropped for me. Luckily I had a kind friend who had an extra that I could trade with.

I got it in the fortune telling room as well but only after I completed the two horseshoe quests.

Me too. I got 3 or 4. Needed them for the quest that required 5 of them. Traded for the rest. Now, I have none since they were taken away after the quest is over.

The book of Necromancy is in the item list of Library at expert level. I haven't it there yet, though.
